Hazlett II of Kentucky and Mary Ann Hazlett-Gaines (Jason) of Kentucky; his grandsons, Ronnie J. Hazlett III and Mason Kelly Hazlett; and his siblings, Kelly Hazlett (Marketta) of Danville, Kentucky, Wendall Hazlett of Danville, Kentucky, Paulette Hyatt of Colorado and Beverly Lay of Danville. He is also survived by several extended family members and close friends.
