He is survived by: his children, Johnny Hornback, Jr. of Louisville, KY, Kay Smith of Bradenton, FL and Karen Michelle Wilson (Marty) of Bowling Green, KY; his sisters, Laverne Carby and Delores Crane of Elizabethtown, KY; his grandchildren, Whitney Davis, Chandler Smith, Kaitlyn Wilson, Ayden Wilson and Kiley Wilson; and his great grandchild Jakai Davis.
