He was predeceased by: his wife Carolyn. He is survived by: his daughters, Susan Warrell (Ben) and Mollie Miller (Sammy); his grandchildren, Megan, Andrew and Paul; his sister Gail Maraman; his son Jeff Green (Marci); and his parents-in-law, Bill and Debbie Molisee. He is also survived by many nephews and nieces, special cousins, friends and family.

In David's memory, donations may be made to Mt. Holly United Methodist Church or Gasper River Catholic Youth Camp and Retreat Center.
