He is survived by: his son Jeff Cook; his brother Cleo Cook; his children, Steven Lee Cook (Angela Biagi Cook) and Judy L. Cook (Russell Lisby); his sister Catherine Smith; his grandchildren, Sarah Beth Cook Gomelsky (Vladi), Meredith Leigh Cook Cornwell (Lary), Christina Lisby Shouse (Mike), Ericka Lisby Gardner (Matthew) and Michelle Lisby; and his great grandchildren, Alex Gomelsky, Max Gomelsky, Leo Vincent Gomelsky, Rachel Ruby Gomelsky, Mallory Cornwell and Madelynn Cornwell.
