She was predeceased by: her father Sherman Temple; and her sister Donna Maness. She is survived by: her husband Michael Cook; her sons, Aaron Dale Cook of Mt. Eden and Allyn Michael Cook of Mt. Eden; her brother David Temple (Lisa) of Franklin County; and her mother Ann Temple of Frankfort.
The family has requested donations in lieu of flowers to assist with the funeral expenses.
