He is survived by: his daughters, Micculla Appleyard (Larry) of Richmond, KY and Angel Smith (Chad) of Madisonville, KY; his siblings, Anna Whitfield of Madisonville, KY, Illene Whitehouse of Madisonville, KY, Patricia Campbell of Memphis, TN, Carolyn Davis of Florida, Margaret Jordan of Florida and Donald David Campbell of White Plains, KY; and his grandchildren, Dawn Marie, Brandon Tyler and Charles Allen. He is also survived by several nieces and nephews.
