She was predeceased by: her parents, Thomas Forker and Mabel Forker; and her siblings, Jewell Foster, Dean Sisk, Loretta Crowley, Helen Tompkins, Raymond Forker and William Forker.
She is survived by: her daughters, Remona Cummings (Terry) and Lisa Proctor (Bobby); her sisters, Jonelle Stone, Katherine Hook and Barbara Hibbs; and her grandchildren, Pamela Boyken, Terry Cummings, Elizabeth Allen and George Proctor. She is also survived by four great grandchildren.
