He is survived by : his wife Diana Perkins of Sebree, KY; and his children, Tim (Misty Collins), Sam Perkins, Jr. of Morganfield, KY, Jonathan Perkins of Sturgis, KY, Racheal (Nick Overfield), Andrea Williams of Evansville, IN and Tracey Lackey. He is also survived by 20 grandchildren 6 great grandchildren.
