He was predeceased by: his parents, Lonzo Wilson Sullivan and Sallie Effie Chandler Sullivan; his wife Gertrude Etta Roberts Sullivan; his daughter Mary Etta Sullivan; and his brothers, James Richard Sullivan, Harrison Jackson Sullivan, Lonzo Sullivan, Jr., Hubert Ray Sullivan and John L. Sullivan.
He is survived by: his sons, Darrell Wade Sullivan of Louisville, KY, Dennis Lee Sullivan (Shelia) of Bowling Green, KY, Daniel Clay Sullivan of Bremerton, WA and Dwight Jeffery Sullivan (Kelly) of Scottsville, KY; his brothers, Billy Edward Sullivan (Virginia) of Aolphus, KY and Paul Bryan Sullivan of Aolphus, KY; his grandchildren, David Sullivan (Angela), Jennifer Vincent (Jeremy), Jeffery, Connor and Dakota Sullivan (Cheyanne); and his great grandchild Hannah Elizabeth Vincent.
