He was predeceased by: his parents, Earl Cardin and Grace Cain Phelps; his sister Nancy Jean; his brother-in-law Vernon Dick; his brother Dale; and his sister-in-law Carol Phelps.
He is survived by: his wife Carolyn Farris Phelps of Science Hill; his daughter Karla (Allen Crawford); his grandsons, Andrew (Amy Crawford), Ben (Brittany Crawford) and Clinton (Shawna Polston); his great grandchildren, Elijah, Teddy Crawford, Veda Crawford, Lilly, Makenna and Trinity Polston; his nieces, Sonya Morris and Susan Trible; and his nephew Bob Phelps.
In lieu of flowers, contributions can be made to your choice of Science Hill Trinity Church, Science Hill Cemetery Association, Inc., or Hospice of Lake Cumberland.
