He is survived by: his children, Joe Kenneth Collinsworth (Ramona) of Salyersville and Jonie Renee Wireman (Tim) of Salyersville; his stepsons, Stacy Puckett of Salyersville and Dwayne Puckett of Tennessee; his siblings, Lonnie Collinsworth of Salyersville, Danny Collinsworth of Salyersville, Gladys Faye Carty of Salyersville, Jean Ellenburg of Salyersville and Murl Marie Howard of Salyersville; his grandchildren, Chase Collinsworth, Devon Watkins, Martika Fyffe, Logan Collinsworth, Breanna Watkins, Lucas Puckett, Michael Wayne Barthelemy and Heather Louise Barthelemy; and his great grandchildren, Lucas Fyffe, Dalton Fyffe, Bearett Kruz Collinsworth, Penelope Nicks Collinsworth, Wilder Hayes Collinsworth, Raylee Ariza, Callahan Weston Collinsworth, Delilah Barthelemy and Ellah Louise Huntley.
