He is survived by: his wife Tammy Smith Collinsworth; his daughters, Amber Nicole Collinsworth (Jayson Hale) of Salyersville and Rebecca Dawn Hayes (Rod) of Mousie; his great-nephew Logan Heath Collinsworth of Salyersville; his siblings, William Ray Collinsworth of Salyersville, Lonnie Collinsworth of Salyersville, Danny Collinsworth of Salyersville, Gladys Fay Carty of Salyersville, Jean Ellenburg of Salyersville and Murl Marie Howard of Salyersville; and his grandchildren, Jeffrey Caleb Combs, Zackray Scott King, Alyssa Kelsey Dawn Campbell, Kirston LaShae Crisp and Cameron Ryan Hale.
