He was predeceased by: his parents, Richard Edward Hayes and Annie J. Hayes; his son Kip D. Hayes; his wife Monna Fay Divine Hayes; and his siblings, Phyllis Annette Coker, Glenda J. Luttrull and James E. Hayes.
He is survived by: his uncles, Don, Donnie, Colonel Hayes and Dr. Hayes; his children, Dr. Don Kevin Hayes and Donita Hayes Rhodes (Tom); and his grandchildren, Dr. Justin R. Hayes and Cameron L. Hayes (Talia).
