He is survived by: his wife Kristin Huemmer Middendorf; his mother Maryjo Fornash; his son Andrew Beyrer (Cory); and his siblings, Lori Adams (Allen), David Fornash (Penny) and Tony Fornash (Doreen). He is also survived by several nieces and nephews.
