He was predeceased by: his parents, Mack Smith and Flora Hunt Smith. He is survived by: his children, Kevin Smith of Russellville, KY and Kimberly Fell of Cox's Creek, KY; and his brothers, Ralph Smith of Lewisburg, KY, Donnie Smith of Lewisburg, KY and Raymond Smith of Greeneville, TN. He is also survived by 5 grand children; 3 great grand children.
As of May 14, 2021, anyone who has received both COVID-19 vaccinations are not required to wear a mask while inside the facility.
