He was predeceased by: his parents, Roy Raymond and Lorelle Marie Mayhew (Knight); his wife Leona Mayhew; his children, Mark Allen Mayhew and Jane Duffe; and his brother Gabriel Patrick Mayhew.
He is survived by: his sisters, Judy Gayle Mayhew of Lexington and Anna Joyce Mayhew of Lexington. He is also survived by seven grandchildren and 3 great grandchildren.
