Middleton (Amy) of Rush, KY; his grandchildren, Alisha Williams (Jason), Jackie Henderson (Lonnie), Patricia Steele (Scotty) and Tiffany Nelson (Marshall); his wife Teresa; his great grandchildren, Makenzie, Hudson, Alex, Peyton, Austin, Savanna, Drew, Maddy, Max, Jacob, Kaitlyn, Kyra, Noah, Oliver, Brian, Ashlyn, Kylie, Spencer, Aurora, Justin, Kelsey, Kendall and Arie; and his siblings, Bobby Joe Middleton (Shirley) of Rush, KY and Delores Hester of Ashland, KY..
