He was predeceased by: his parents, James Alvin "Turkey" and Mary Elizabeth Cates Middleton. He is survived by: his wife Melinda Ann Middleton; his son Ricky Alvin Middleton (Pam Grant); his stepchildren, Josh Robinson, Brandy Thomas, Dana Roberts and Amber Murphy (Jake); his siblings, Jamie Rhodus, Phyllis Sproles (Wayne) and Jimmy Douglas Middleton (Teresa); and his grandchildren, Makenzie King, Grant King, Jonah Murphy, Jayden Murphy, Noah Murphy, Skyler Murphy, Emily Isaacs, Landon Isaacs, Kylie Ann Robinson, Ellie Robinson, Luke Roberts and Lane Roberts. He is also survived by several nieces, nephews, cousins, other relatives and friends.
