He was predeceased by: his parents, Jeffrey Hayes and Lillian Kluckner Hayes. He is survived by: his brother Craig Hayes of Mammouth; his nephew Jeffrey T. Haywell (Krystina) of Morris; his niece Danielle C. Sullivan (Brent) of Milwaukie; his former wife Karen Dilka; his girlfriend Paula Bowman of Richmond, Kentucky.; his partner's children, Brian Bowman (Michele) and Brooke Mitchell (Chris); and his grandchild Laurence.

In lieu of flowers, memorial donations may be made to the EKU Tom King Scholarship Fund.
