He was predeceased by: his father David; and his sister Kimberly. He is survived by: his daughters, Jenny and Stephanie (Casey); his grandchildren, Tré and James; his mother Janet; his sister Kelly; and the mother of his children Sandy.
Mr. Hazelwood's final act of generosity was the gift of life, saving three individuals through organ donation.
