She is survived by: her son Anthony Hazelwood (Venitta) of Parkersburg, WV; her siblings, Shirley Rice of Greenbrier, AR, Linda Hazelwood of Radcliff, KY, Tammy Butler (Danny) of Sonora, KY, Terry Hazelwood of LaGrange, KY and Steven Hazelwood of LaGrange, KY; her grandchildren, Samantha, Justin and Zane; and her nephew Wesley Hibbs of Radcliff.
