He was predeceased by: his parents, Harvey Kenneth and Helen Shultz Cook of Virginia; his former wife Conna Jo Hannon Cook; and his siblings, Harold, Dave, Jerry Cook, Dona Cooper and Arnetta Burgess.
He is survived by: his spouse Dama Carol Armstrong Cook; his children, Carey Dee Cook, Bret Daniel Cook (Michelle), Doug Arnold (Susan), Greg Arnold (Jackie) and Misty Wise (Jerry); and his sister Virginia Bales (Gene). He is also survived by 13 grandchildren, 3 great grandchildren.
