He was predeceased by: his parents, Howard Thomas (H. T.) and Wenona Ruth Proctor Lewis; his wife Careyon; and his son Ryan. He is survived by: his children, Eric Lewis (Gayla) and Tara Leigh Gaines (Rusty); his sister Becky Duvall (Donald); and his grandchildren, Audrey and Brandon Lewis.

Burial will follow in the Owenton Cemetery and memorials may be made to Beka's Toppers or the Chase Duvall Scholarship Fund.
