He was predeceased by: his parents, Willis Mason and Geneva Mefford Curtis; and his brothers, Larry David Curtis and Glenn Louis Curtis. He is survived by: his wife Thelma Curtis; his son Jeremy Dale "JD" Curtis; his sister Anita Prows (Scott); his children, Tony Curtis (Georgia), Micah Curtis (Tiffany), Kimberly Gose (Butch) and Melissa Curtis; his grandchildren, Dustin Curtis (Angie) and Mindy (Anthony); his sons, Michael Warren, Cory Warren, Lane Curtis, Cody Curtis and Kelly Curtis; and his great grandchildren, Landon Smith, Rylie Smith, Corbin Smith, Grayson Smith and Jacob. He is also survived by Sons; and a host of nieces, nephews, and cousins.
