He is survived by: his wife Barbara Cook (Griffin); his children, Darlene Wainscott (Mike) of Owenton, KY and Phil Cook of Owenton, KY; his sisters, Mary Lee Tackett and Alice Margaret Wheeler of Monterey, KY; his grandchildren, Deborah Parker (Rodney), Michelle Dunavent (Brad), Shannon Mitchell Cook, Jason Wainscott (Marlene) and James Phillip "JP" Cook (Kristina Ueltschi); and his great grandchildren, Kirsten Parker, Luke Dunavent, Kaitlyn Parker, Savannah Anderson, Lucy Dunavent, Wade Cook, Brennan James Cook and Memphis Cook.
