He was predeceased by: his father John Thomas Forsythe; and his brother Anthony Lee Forsythe. He is survived by: his mother Nancy Aline James Forsythe; his wife Freda Renee Forsythe; his sons, Thomas Forsythe (Kasey), Patrick Caskey (Lindsey) and Nick Forsythe (Emily); his stepchildren, Amber Craddock (Keith), Karl Vandergriff, James Dennis and Brandon Dennis; his sister Kay Canler; his aunt Linda George; his sister-in-law Pam Forsythe; and his nieces and nephews, Tony and Joey Forsythe. He is also survived by 15 grandchildren; one great-grandchild.
