He was predeceased by: his parents, Edmund Ballard and Mary Howard Ballard; his siblings, Mary Ellen Brown (Tony) and Raymond Ballard; and his sister-in-law Barbara Ballard.

He is survived by: his wife Carolyn Ballard; his children, Karen Ballard, Stephen Ballard, Jr. (Katherine Ballard-Mullen), April Ballard (Cody) and Mary Alget (Shawn); his grandchildren, Sarah Head, Austin Head, Laura Head, Dawson Johnson, Kinzley Johnson and Liam Reynolds; his great grandchildren, Anderson Brinker and Amber Whiteaker; his siblings, Mike Ballard, Wayne Ballard (Linda), Nancy O'Bryan (Richard), Anna Houghton (Bill), Marie Blumeier (Don) and Helen Cox (Ken); and the mother of his children Roxanne Ballard. He is also survived by several nieces and nephews.
