He was predeceased by: his parents, D. Donald Mayfield and Emogene Smith Mayfield. He is survived by: his wife Jo Etta Smith Mayfield; his children, April Mayfield of Owensboro, Stephen S. Mayfield of Owensboro, Donald Wayne Benton (Amy) of Lenoir, N. C. and Chris Welch of Burleson; his grandchildren, Allie Lewis of Owensboro, Amber, Emily Benton of Lenoir, Ashley, William and Haley Welch of Burleson; and his sisters, Donna Faye Wiernik (Eric) of Tennessee and Lisa Cox (Roger) of Owensboro. He is also survived by one niece and two nephews.
