"Jay" Hazel Jr. (Paula) of Knottsville and Joelle Hazel Leali (Robert) of Jupiter, Florida; his stepdaughter Joyce Borum Grant of Philpot; his grandchildren, Jaclyn, Joshua, J. T. Hazel, Jenna, Josie and Nikki Leali; his stepgrandchildren, Bailey, Garrett Henderson and Sydney; his great grandchild Jayden Hazel; his sister Nanette Hazel of Louisville; and his nephew Zach Callery (Beth).
Expressions of sympathy may take the form of contributions to Gateway Foundation, St. Williams Church, or the AA Central Office in Owensboro.
