He was predeceased by: his parents, Randolf Lane Cook and Eva Mae Bates Cook; his sister Margaret Casey; and his brother-in-law George Casey. He is survived by: his wife Udella Blackburn Cook of North Middletown, KY; his children, David L. "Dadoo" Cook Jr. (Ruth Marie) of Belfry, KY and Wendy Lane Cook of Belfry, KY; his grandchildren, Mason of Belfry, KY and Ranzee Cook of Belfry, KY.; and his close friends, Charlie Paterino, Ronald "Grady" Linville and Albert Allen.
