He was predeceased by: his parents, Louis Leaf and Marie Bevington Leaf; and his siblings, Garnett, Louise, Carolyn, Jean and Charles Leaf.

He is survived by: his wife Sue Leaf; his son Mark Leaf; and his grandson Alex.

In lieu of flowers, memorial contributions may be made to Hospice of the Bluegrass Care Navigators.
