He was predeceased by: his father Franklin D. Jervis. He is survived by: his wife Nanci Jervis; his mother Shirley Anne Green Jervis; his children, Sarah Hillenmeyer (Chase) and Jared Jervis; and his grandchildren, Henry, Graham and Caroline.

In lieu of flowers, memorial contributions may be made to Hospice of the Bluegrass or the Lexington Rescue Mission.
