She was predeceased by: her parents, John H. Hepner and E. Mildred Hepner (Stebbins); and her brother Edward O. Hepner. She is survived by: her husband Charlie; her children, Mandy Slack (Robert Baker) of Westerville, OH and Howard Slack (Gina) of Nicholasville, KY; her grandchildren, Katie, Lucy Baker, Nate, Asher and Ruby Slack; her brother John Hepner Jr. (Kathleen); and her sister-in-law Linda Hepner. She is also survived by numerous nieces and nephews.

In lieu of flowers, donations can be made to Providence Christian Church or the Alzheimer's Association in memory of Cathie Slack.
