He was predeceased by: his parents, Roas Underwood and Myrtle Purvis Underwood; and his siblings, Hardin Underwood, Martha Perry and Ruby Hager.
He is survived by: his wife Mary Blackburn Underwood; his sons, Alvin Mark Underwood (Jae) and Leonard Welch Underwood; his grandchildren, Lindsey Ryan Baker (Willie), Rebekah Lauren Hoffman (Corey), Emma Rose Underwood and Elliott Roas Underwood; his great grandchildren, Cody Ryland Eads, Wiley Griffin Baker, Raylee Jackson Hoffman, Ryker Vincent Hoffman and Rockland Mack Hoffman; and his brother Charles Underwood (Marcella).
