He was predeceased by: his siblings, Timothy Phillips and Rebecca Phillips; and his nephew Wyatt Whitaker. He is survived by: his children, Jeremy David Phillips of Clarksville, Stephanie Rush (Greg) of Bowling Green, Zachary Phillips (Erin), San Antonio and Jacob Ross Phillips of Madisonville; his grandchildren, Tyler, Morgyn, Seth, Jackson, Dylan, Colton and Jett; and his siblings, John Phillips (Kathy) of Nortonville, Mark Phillips (Peggy) of Henderson, Mary Sue Whitaker (Ricky) of White Plains and Debi Stratton (Kevin) of Nortonville.
In lieu of flowers the request that monetary donations be made to the Wyatt Whitaker Athletic Scholarship Foundation at First State Bank in Central City.
