He was predeceased by: his parents, Harry D. Cates and Ruth Cates; and his siblings, Helen Hartline, Margaret Cates, Finis Cates, Marvin Cates and James Cates.
He is survived by: his wife Linda Cates of Nebo, KY; his children, Sandra Zigler (Kevin) of Providence, KY, David Cates II (Cheryl) of Mortons Gap, KY and Patrick DeWayne Cates (Charlotte) of Clarksville, TN; and his siblings, Annie Mae Boze of Madisonville, KY, Barbara Lamb of Nebo, KY and Tommy Cates of Henderson, KY. He is also survived by several grandchildren; several great grandchildren; and several nieces and nephews.
