He is survived by: his wife Diane Puckett of Nebo, KY; his children, Teresa Hearrin of Madisonville, KY and Matthew Puckett of Dixon, KY; and his sisters, Barbara Halterman of Sturgis, KY and Diann McClain of Henderson, KY. He is also survived by 6 grandchildren and 1 great grandchild; several nieces and nephews.
