He is survived by: his children, Chandra Pendley (Troy) of Nebo, KY and Chris Mayhew (Donna) of Madisonville, KY; his siblings, Jimmy Mayhew of Slemp, KY and Ruth Martin of Nacogdoches, TX; his granddaughters, Madeline Lumpkin (Jonathan) of Madisonville, Miriah Pendley of White Plains, KY and Bella Mayhew of Beechmont, KY; and his great grandchildren, Heather, Courtney, Casey, Emma, Kandice, Aidan and Wyatt.
