V. and Patrick Cook (Leila) of London, KY; his grandchildren, John Matthew Stephens, Kourtney Paige Stephens and Hanna Nicole Cook; his great grandchildren, Kieran Joseph Stephens and Kashton Raydan Stephens; his sister Dean Jung of Cincinnati, OH; his close friend Mikey Epperson of Nancy, KY; his former wife Corine Cook of Harlan, KY; and also James Epperson (Wanda). He is also survived by cousins and other family members.
