He was predeceased by: his parents, David O'Keefe and Nancy Kurnica O'Keefe; and his son Scott Michael O'Keefe. He is survived by: his wife Mary O'Keefe of Murray; his daughter Stephanie Michalak (Jarrod) of Colorado; and his grandchildren, Mason, Aubree and Remington.
