He was predeceased by: his son Anthony Willoughby; his grandparents, Howard, Annie Willoughby, Bob and Mary Costella; his brother Willard Willoughby; and his niece Tabitha Bellamy.
He is survived by: his parents, Lenzie Willoughby and Mary Costella Willoughby of Jeffersonville, KY; his children, Brandon Willoughby of Jeffersonville, KY, Jeffery A. Willoughby of Mt. Sterling, KY and Haley Willoughby of Mt. Sterling, KY; his sister Joyce Willoughby of Mt. Sterling, KY; and his grandchildren, Steven Willoughby, Gracie Willoughby, Tyler Willoughby, Jackson Willoughby, Olivia Willoughby and Ellie Mae Willoughby.
