He was predeceased by: his mother Goldie Polly. He is survived by: his significant other Lisa Lucas; his daughters, Cari Polly and Kayla Polly; his father Decoursey Polly; and his siblings, Mike Polly, Atha Brewer (Shawn), Ralph O'Brien and Linda McGuire (Mike). He is also survived by many aunts, uncles, nieces and nephews; as well as many more family and friends.
