It is with deep sorrow that we announce the death of Hazel Dick (Monticello, Kentucky), who passed away on October 27, 2022, at the age of 84, leaving to mourn family and friends. You can send your sympathy in the guestbook provided and share it with the family.
She was predeceased by: her parents, Delbert Koontz and Dora Koontz (Barnes); her husband Donald Dick; and her siblings, Roy Edward, James Wilroe, Arvel Benton Koontz, Earlene Barnett, Lorene Barnett, Cleo Coffey, Phyllis Neely, Gwen Morrow and Debra Koontz.
She is survived by: her children, Darrell Dick and Sherry Anderson; and her brother Gary Lee Koontz (Julie). She is also survived by four grandchildren and several great grandchildren.
