She is survived by: her nephew David Cook (Laura) of Middlesboro, KY; her niece Lori Blair (Don) of Gainesville, GA; her great-niece Erin Bell (Rob) of Louisville, KY; her great-nephews, Alex Cook (Jessica) of Middlesboro, KY and Daniel Blair (Kalee) of Bogart, GA; her cousins, Katherine Cook Thomas of Knoxville, TN and Ann Simmons Parks of Florence, SC.; and also, Lauren, Kate Bell of Louisville, KY, Grayson and Sawyer Cook of Middlesboro, KY.
