He was predeceased by: his parents, Otis Burle Collins and Dolores Franklin Munoz; his sons, Benjamin Collins and Jeremiah Siebert; and his brother David Harrell.
He is survived by: his children, Otis Collins III of Liberty, Jazmine Collins of Liberty, Sean Collins of Russell Springs, KY, Victor Collins of Liberty, Violet Collins of Goose Creek, S. C., Shady Saldivar of Florida and Josh Jones of Liberty, Twenty; and his brothers, John Kelsey of Florida and Thomas Thrasher of New Mexico.
Due to efforts to combat the spread of COVID-19 and New requirements implemented by the Office of Governor Andy Beshear and The Funeral Directors Association of Kentucky, all funeral services will be performed privately.
