He was predeceased by: his parents, Orville Forman and Lottie Presley Forman. He is survived by: his wife Barbara Taulbee Forman; his daughters, Bobbie Jo Osborne of Ohio, Leah Crist of Alexandria and Sheila Peck of Tollesboro; and his stepchildren, Tina Fannin of Vanceburg, Debbie Hall of Sciotoville, Randy Taulbee of Brooksville and Dallas Stevenson of Garrison.
