He is survived by: his wife Angie Davis Boaz; his daughters, Jen Boaz Davis of Henderson, KY and Brittany Boaz Titzer (Hank) of Henderson, KY; his stepchildren, Kate Robertson of Aurora, KY, Matthew Robertson (Amanda) of Aurora, KY and Benjamin Robertson of Aurora, KY; and his sister Mary Boaz (John Perry) of Louisville, KY. He is also survived by nine grandchildren.
