He is survived by: his wife Martha Sullivan; his children, Gary Sullivan (Teresa) of Mayfield, KY, Terry Crouch of Mayfield, KY, Tina Sullivan of Mayfield, KY, Donna Bell (Ricky) of Mayfield, KY and Matthew Blake Sullivan (Hailey) of Mayfield, KY; and his siblings, David Sullivan (Debbie) of Mayfield, KY, Carolyn Sullivan of Mayfield, KY and Ruth Ann Thompson (Jimmy) of Fancy Farm, KY. He is also survived by 10 grandchildren.
