He was predeceased by: his father Harlan Thomas Stevens; and his sister Sharon Holland. He is survived by: his mother Mable Dowdy Stevens of Mayfield, KY; his children, John Thomas Stevens (Kortnee) of Wingo, KY and Cari Mikez of Mayfield, KY; his sister Angela Todd (Keith) of Kirksey, KY; and his grandchildren, Aiden Sanderson, Kenlee Bushart and Kadince Bushart. He is also survived by several nieces and nephews.
