He is survived by: his children, Nicole Terrell (Tad), Brad Sullivan (Erin) and Jay Sullivan (Valerie); his grandchildren, Kayla Hazlewood (Austin), Emily Klapp, Trevor Terrell, Jack Sullivan, Liam Sullivan and Reese Sullivan; and his great grandchildren, Emmett Hazlewood and Isla Hazlewood.
